Give me a call. I will pick up the phone myself.  No operators or middlemen. I'll give you my advice based on my experience. Maybe that's all you need.  If you would like to give me some soundbites of your story, before we talk, fill out the  MY STORY form to expedite the call. You don't need any documentation or plan of action for our initial conversation, just your current situation and concerns. I'm here to guide you through the process and let you know what I think you'll need based on the needs of the person who requires help. From there, we'll consider your budget, timeline, location preference, and expectations to come up with a plan moving forward.

There will likely be a couple of options that make the cut.  I will work with your schedule and tour these facilities with you.  Remember, these places are going to try their best to "sell" their home to you.  I'll be there to make sure we ask the right questions and ensure there is a proper fit.

2. Get the proper documentation

​Whichever Living Option you select moving forward, they will require certain documentation as part of the admittance process.  Most important are a current Physician Report and Medication List, but you will also want a POLST, PoA, and several others.  Don't worry!  I'll help you get the required paperwork in order. ​ With this paperwork in hand, I'll want to speak with the loved one who is getting a new home as well as their most recent caregiver and/or the closest person who best knows their condition(s) and daily routines.   During this process, we will best assess their physical and mental abilities and determine their care needs.  I also want to get a feel for their personality, likes and dislikes, and overall feel for what will make them most comfortable in their new home.  ​ Once I have completed my profile, I can begin the placement search for the best Options in the Carmichael/Arden area that meet our agreed standards and criteria.  It may be the case that there are no openings or options available within the geographic area that I specialize in.  In this case, I will refer you to one of my trusted partners in the outlying areas of Folsom, Rocklin, Sacramento, or Rancho Cordova. Either way, I am very diligent when recommending a place or solution that best fits the resident.

4. Choose your Community

Time to make a decision

Once you have seen the best options available, it's time to make a choice.  Do you want to continue on your current path?  How safe is that option?  How sustainable is it? Or, are you ready to make the move to a different living environment?  If so, which option did you like the best?  I'll contact the Home that you have decided and help negotiate the best terms of the Agreement.  We'll want to be sure that all the details of care are included in the Agreement and that a system of controls is in place so that you can verify that the level of care that has been detailed in the Agreement is actually being provided.
